"Personal Information Supplied From Request"
 

Question:
I received an MVR with the message: "Personal Information Supplied From Request". What does this mean? 

 
Answer:
This means that the source from which iiX obtained the MVR information has placed restrictions on the dissemination of the driver's personal information - i.e. Name, DL#, DOB, and Address.  Therefore, the Name, DL#, SSN, and other personal information on the report were taken from the request that you sent to iiX. They are not necessarily the Name, DL#, SSN, or DOB that was returned by the state.
 

On any MVR that carries this message, iiX will perform a Request Match Analysis and indicate (in the Miscellaneous and State Specific section of the MVR) whether the First Name (FN), Last Name (LN), DL# (DL), and Date of Birth (DOB) that you submitted matched the information provided by the state.
