AMSEXPW Caused a General Protection Fault in SSFORMFX.VBX
 

Problem:

The following error message is displayed while attempting to enter new requests or view System Information:

AMSEXPW Caused a General Protection Fault in SSFORMFX.VBX

Solution 1:

Verify the date and time stamp on the SSFORMFX.VBX file in the \WINDOWS\SYSTEM directory. It should be at least 5/23/94.  If the file has any other date, try renaming the file and reinstalling Express for Windows.  Express will install another copy of the file.
 

Solution 2:

Check for multiple copies of the SSFORMFX.VBX file. Use the command :

  • DIR SSFORMFX.VBX /S

at the root of the drive from which Windows® is launched, - usually C:

or

Use the Find option in Windows®.

If multiple copies of the file are found, rename any copies of the file that are not located in the Windows System directory ( usually C:\WINDOWS\SYSTEM).
 

Solution 3:

Check the system date by changing to a DOS prompt and typing DATE. The current date should display. Make sure it is correct. Years after 2063 are known to cause this error.
 

Solution 4:

The SSFORMFX.VBX file may be corrupt. Try renaming the existing copy and reinstalling Express.
 

Solution 5:

If the system date format in Control Panel | Regional Settings is set to M/D/YYYY, try setting it to MM/DD/YYYY.
