DESIGNATION OF iiX AS AN EMPLOYER PULL NOTICE (EPN) AGENT
Please note, California has different requirements regarding the monitoring process and how records are obtained and secured as compared to other states. Please maintain the EPN record for only as long as your business purpose requires. Employers choosing to authorize iiX as their agent to administer the California EPN program should read and follow the information and instructions outlined below. They provide helpful information about the California EPN program and offer links to various forms required by the State of California Department of Motor Vehicles. Before designating iiX as your authorized EPN Agent, please consider several items.
New EPN Users:
Employers who use the California EPN program must possess a state-issued "Requester Code" for the EPN program. If the employer has not yet acquired a state-issued "Requester Code" for use with the EPN program, the EMPLOYER PULL NOTICE PROGRAM APPLICATION, Form INF 1104, and the PULL NOTICE CONTRACT, Form INF 1105, should be completed and mailed as instructed on the forms.
- The State of California requires (Vehicle Code Section 1808.1) that certain drivers be enrolled in the EPN program. If you have a mix of mandated and non-mandated drivers, all can be included in the EPN program. The inclusion of non-mandated drivers requires that each such driver complete the AUTHORIZATION FOR RELEASE OF DRIVER RECORD INFORMATION, Form INF 1101. This form must be retained at the employer's principal place of business and made available upon request to DMV staff. NOTE: The State of California created the EMPLOYER PULL NOTICE PROGRAM ENROLLMENT PACKET, Form INF 1250A. This packet contains all California EPN forms mentioned above, with the exception of EMPLOYER PULL NOTICE PROGRAM AGENT AUTHORIZATION, Form INF 2110, which also is required.
- Once an employer possesses a California "Requester Code" for the EPN program, the employer may complete Form INF 2110. This form will designate iiX as the employer's choice of authorized EPN Agents. The California EPN Program places certain requirements on employers. To see them, please click here.
- The Employer must enter its name exactly as it is recorded with the EPN Program office. Failure to do so can cause delays. Please note: if the information on record should be changed, the INF 4 form needs to be filled out and accompany the INF 2110 form when sent to iiX.
- After completing Form INF 2110, please forward the original copy with the "wet" signature by mail to:

Once iiX has received your completed Form INF 2110, we will complete the "EPN Agent" portion and forward the original, by Federal Express standard, to the EPN program office for processing. Once the EPN Program office receives the completed form, the following will occur:
- The EPN Program office will verify that the employer is currently set up with an EPN requester code, and hand off to the EPN IT department for set-up.
- The EPN IT department will electronically establish the new EPN Agent/Employer relationship.
- Once the internal set-up has been completed, iiX will receive notice the employer set-up has been completed.
- Once iiX has received notice, the employer will be notified and drivers can be added and maintained through ExpressNet's add/delete option for Driver Advisor. For more information on this process, see the ADDING/REMOVING DRIVERS TO EPN DRIVER FILE section below.
Current EPN Users:
- Employers wishing to designate iiX as their EPN Agent should obtain a billing statement from the EPN Program office to make sure there are no outstanding balances. All unpaid balances, if any, must be handled with the EPN Program office before an EPN Agent change can occur. The EPN office contact number is (916)657-6346.
- Employers possessing a California "Requester Code" for the EPN program and have addressed all unpaid balances with the EPN Program office may complete the EMPLOYER PULL NOTICE PROGRAM AGENT AUTHORIZATION, Form INF 2110. This form will designate iiX as the employer's choice of authorized EPN Agents. The California EPN Program places certain requirements on employers. To see them, please click here.
- The Employer must enter its name exactly as it is appears as it is recorded with the EPN Program office. This information can be located on an EPN MVR printout. Failure to do so can cause delays. Please note: if the information on record should be changed, the INF 4 form needs to be filled out and accompany the INF 2110 form when sent in to iiX.
- The employer must terminate the relationship with the current EPN agent by forwarding a notice of termination to the current EPN agent. The notice should provide thirty (30) days notice of cancellation to allow the current EPN agent to properly communicate with the EPN office. Please forward a copy of the termination notice to the current EPN agent and iiX to facilitate appropriate co-ordination with the EPN office.
- After completing Form INF 2110, please forward the original copy with the "wet" signature by mail to:

Once iiX has received your completed Form INF 2110, we will complete the "EPN Agent" portion and forward, by Federal Express standard, the original to the EPN program office for processing, along with the copy of your notice of termination with the current EPN agent as close to the scheduled termination date as possible.
Note: The current EPN agent must contact the EPN office 1-2 days before the scheduled termination date to verify cancellation of services.
Once the EPN Program office receives the completed form, the following will occur:
- The EPN Program office will verify that the employer is current with billing and other items concerning the employer, and hand off to the EPN IT department for set-up.
- The EPN IT department will electronically establish the new EPN Agent/Employer relationship.
- Once the internal set-up has been completed, iiX will receive a transfer file containing the employer's driver list. Note: Should there be any time period that elapses between current agent termination and iiX receiving the first transfer file, any EPN notices generated will be forwarded to you via U.S. Post Office by the EPN office.
- Once iiX has received the employer's driver list, we will begin receiving driver activity on the next business/processing day.
ADDING/REMOVING DRIVERS TO EPN DRIVER FILE
There are two methods of adding or removing drivers from the EPN program as follows:
California Licensed Drivers:
- Employers wishing to add/remove drivers that possess a California driver's license should use ExpressNet's add/delete option for DriverAdvisor. These changes are communicated from the iiX platform to the California EPN system automatically.
- Best practices call for drivers to be added to DriverAdvisor once they have been formally hired, and that drivers be deleted from DriverAdvisor upon termination from employment.
Out-of-State Licensed Drivers:
EPN PROGRAM EMPLOYER ACCOUNT CHANGES
If an employer needs to update/change its account information with the EPN Program office, it should complete Form INF 4:
- To be used when any changes occur to the company name, address, phone number, or contact person for the employer enrolled in the EPN Program.
After completing Form INF 4, please forward the original copy with the "wet" signature by mail to:

Once iiX has received your completed Form INF 4, we will forward, by Federal Express standard, the original to the EPN program office for processing
Must be submitted by employer within 10 days of change.
EPN PROGRAM EMPLOYER RESPONSIBILTIES
Employer acknowledges and understands that:
- Utilizing an EPN Agent does not absolve the Employer of any responsibility for compliance with the provisions described in the CVC Section 1808.1.
- Hereinafter all record information will be exclusively forwarded to the EPN Agent, Employer will not have the option of requesting record information to be sent to the Employer's place of business until this authorization is rescinded.
- Upon obtaining the services of an EPN Agent, Employer must notify DMV (by telephone or fax) and request a billing statement of all unpaid charges. EPN Agent will then submit total payment of unpaid balance, on Employer's behalf, enabling the EPN Agent to begin with a new billing cycle and balance.
- Employer agrees to notify DMV, in writing, at least 2 weeks prior to terminating the services of the EPN Agent. Upon receipt of notification all record information will revert back to the Employer's address of record.
- Employer acknowledges that DMV records contain restricted information, and Employer has read and understands the provisions of CVC Section 1808.47.
- Employer and EPN Agent understands and acknowledges that any failure to maintain confidentiality is both civilly and criminally punishable pursuant to CVC Sections 1808.45 and 1808.46.
- Employer understands, acknowledges and agrees to absolve and hold harmless the department in any failure on the part of the EPN Agent's non-compliance of agreements established as a result of this process.
- Employer understands, acknowledges and agrees to absolve and hold harmless the department in any failure on the part of the EPN Agent for non-receipt of driver record information in a timely manner.
- Should the Department of Motor Vehicles (DMV) cancel, revoke, or terminate the EPN Agent's authority to obtain DMV records, the EPN Agent/Employer relationship shall immediately be dissolved and EPN Agent must return all DMV records back to the Employer's address of record within 48 hours.
- EPN Agent will submit this Authorization form to DMV, and a copy of the Authorization form containing the Employer's signature must be retained at the Employer's place of business, and made available upon request to DMV Audit staff and the California Highway Patrol.
- DMV record information must be presented in its entirety either in the pre-approved hardcopy format as provided by DMV to the EPN Agent, or if electronic, the date and unique identifier of the individual reviewing the driver record must be placed on the electronic records received from DMV at the time first reviewed. The identifiers must be permanent and unalterable.
